Brazil's public healthcare system (SUS) is free for everyone including tourists, but it's severely overcrowded. Private hospitals in São Paulo and Rio de Janeiro (Hospital Albert Einstein, Hospital Sírio-Libanês) are world-class. Quality varies enormously by region.
Private hospital visit: $150–500. Hospital day: $500–2,000 at top São Paulo hospitals. Private ambulance: $200–500. The gap between public (free but overwhelmed) and private (excellent but expensive) care is enormous.

Dengue, Zika, and chikungunya are endemic — especially in northern and northeastern Brazil. Yellow fever vaccination is required for some regions (Amazon basin). Street crime is a major concern in Rio de Janeiro, São Paulo, Salvador, and Recife. Car accidents and motorcycle injuries are very common.

$100,000+ medical coverage. Evacuation coverage if visiting the Amazon or remote areas. Trip interruption coverage for domestic flight delays (common in Brazil). Theft/robbery coverage — Brazil has high rates of smartphone theft and mugging.
