Good healthcare system; insurance recommended for private facilities. As a European destination, travel insurance requirements depend on your nationality. EU/EEA citizens benefit from the European Health Insurance Card (EHIC), which covers necessary medical treatment, but EHIC does not cover repatriation, mountain rescue, or trip cancellation. Schengen visa applicants must show insurance with minimum €30,000 medical coverage.
GP visit: €30–60. Emergency room: €100–300. Hospital day: €300–800. Spain uses a similar system to France — pay upfront, seek reimbursement with EHIC. Tourist area private clinics charge premium rates.

Pickpocketing — Barcelona is Europe's pickpocket capital (La Rambla, Metro, beach). Madrid (Sol, Gran Vía) is also high-risk. Running of the Bulls in Pamplona (July) causes injuries annually. Heatwave deaths in summer (40°C+ in southern Spain). Beach drownings from strong Mediterranean currents. Ibiza party scene — alcohol and drug-related incidents.

Barcelona pickpockets are professionals — use a money belt, not a backpack. Running of the Bulls (San Fermín, Pamplona, July 6–14) is extremely dangerous and may be excluded by your insurer — check before participating. Summer heatwaves kill people in Spain every year — stay hydrated and avoid midday sun. Ibiza medical incidents from recreational drug use are common and may not be covered if drugs are involved.
